We use a Librium ( Chlordiazepoxide) detoxification for the Alcohol detoxification, and also commence a Lofexidine, Methadone or Subutex -Opiate detox, whichever is appropriate for the Client.[ please ‘click’ on Opiate link below to view the Lofexidine & Methadone detox details ]
The Client will be assessed pre- admission and then again prior to commencing Detox, to determine the appropriate Librium dose, based on the Client’s withdrawal symptoms, utilising CIWA-Ar (Clinical Institute Withdrawal Assessment of Alcohol Scale) the Client’s height, weight and how much Alcohol is normally used by the Client.
What it covers
This will cover both Alcohol and Benzodiazepine withdrawal. If there is a medical reason for not using Librium (e.g.the Client has liver failure) our G.P’s will prescribe suitable alternatives. Once the Alcohol and Opiate detox is completed, the Client is assessed as to whether there is a need for further benzodiazepine detox. If required, the Benzodiazepine detoxification would continue using Diazepam with Carbamazepine (Tegretol) to safeguard against Benzodiazepine withdrawal seizures.
In addition to the above, all clients admitted for detox will have the following medication: Pabrinex I.M. daily for 5 days followed by Thiamine and Vitamin B co strong daily for the remainder of the detox period and for 2 weeks after discharge. All females are pregnancy tested prior to any dose of any medication being given. If a female Client is pregnant and this is known prior to the Client being admitted, a letter from the client’s Consultant Obstetrician will be required to confirm that it is safe to commence detoxification prior to the client’s admission.
